Rules and regulations for patients of Genesee County Free Medical Clinic
Patients must be seen by a clinic doctor (at minimum) every six (6) months.
To receive ongoing care/receive referrals,
PROOF OF INCOME
must be provided yearly or you will be considered non-compliant thus reducing the services or medication we can provide for you. If you do not file your taxes, you will be asked to sign a 4506-T form. Three in-clinic visits are required prior to referrals outside of our clinic unless deemed emergent by GCFMC’s Medical Director.
For an emergency, if the clinic is closed, you can go to the Hurley Emergency Room. Please note that you will receive a bill; however, you may qualify for a discount of up to 85% on hospital charges and up to 60% on physician charges, depending on your Federal Poverty Level (FPL) qualifications. If you are treated in the emergency room, you may be contacted by ADVMAS who can help with Medicaid even if you have been denied. In addition, Hurley offers interest-free payment plans on self-pay balances. For assistance or questions, please contact their Customer Service team at 810-262-9255.
Three (3) No call NO shows will result in discontinuation of services at the Free Clinic.
When you have one (1) week of medications left, please call the clinic and request a refill, keep in mind the pharmacist is only at the clinic on Tuesdays, Wednesday afternoons and Friday mornings. If you do not have refills available you will be expected to make an appointment to see the Doctor.
If we make you an appointment with a specialty Doctor, you MUST keep the appointment, in case of an emergency, it is your responsibility to call that specialty Dr and reschedule with them as soon as you are aware that you will not be able to make the scheduled appointment with them. Failure to do so will result in no referrals for six (6) months.
If you need prescription assistance through Patient assistance and do not comply with the needed documents to process your patient assistance application you will be without medications.
The clinic reserves the right to discharge you as a patient of the Genesee County Free Medical clinic if you fail to comply with the Doctors orders.
Within 30 days of becoming a patient of the Genesee County Free Medical Clinic, we ask that you make an appointment with Our Genesee Health Plan rep Regina (810) 232-7740 EXT 240 to apply for GHP.
In an effort to encourage healthy behaviors and lifestyles you may be asked for authorization for use of disclosure of protected health information for photography, education, marketing, fundraising, and public relations purposes. Please indicate your approval for promotion of positive health outcomes.
